How does it work?

 

If your child’s school offers the All Kids School-Based dental services, forms will be sent home. You may also contact your child’s school nurse to see if this is a provided service. Any student that brings back a completed consent form by their parent or guardian will receive services free of charge. If you have insurance, your information is necessary, however, no payments will be due from you.

 

Step by Step:

 

• Coordination between the school nurse and our dental team to arrange the school visit.
• The school will distribute forms to each student to take home. Parents or guardians will review, complete, and return the forms to the school.
• Our team will set up a portable dental clinic in the designated area and provide preventative dental services.
• A follow-up report will be sent home to parents, and the State of Illinois Dental Examination forms will be given directly to the school.
